Join a 30+ year old, progressive, physician run group specializing in managing the care of a predominately Geriatric population with over 90 offices.

A leader in preventive, value-based care we are seeking physicians and clinicians who appreciate senior patients, excel in a collaborative environment, and strive for positive outcomes for their patients. The group’s focus is on outcomes, not on volume of patients.

Details:

• Physician run organization with peer collaboration
• 1st visit patients are scheduled at 40 minute intervals and follow ups are 20 minutes
• On average the daily schedule is less than 20 patients / day
• W/L balance
• Monday – Friday
• Practice coverage – telephone 1 week every 6 months
• Exclusive focus on complex senior patients
• Small panel size is:  225 patients
