Overview:

• Established Practice and Patient Panel

• 40 hours per week with 36 patient contact hours and 4 hours of administrative time built in.
• Looking for flexibility to increase patient access
• Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination

Responsibilities:

• Average patients/day: 22-24
• Provide Supervision of APPs
• Pooled phone call triaged through my-nurse call center: 1 week on every 5-6 weeks
• Additional call 4 weeks per year covering Pediatric Admitting at the Hospital
• Excellent communication (written and verbal), analytical, and problem-solving skills
• Strong leadership skills required to set the daily tone of the clinic
• Dedication to highest tier of patient experience and quality metrics
