Geriatrics Group provides office-based consultation services for older adults at risk for or with complex chronic and age-related conditions. Consultation is done in the context of each individuals functional and psychosocial situation, taking into account their goals of care and wishes for the future.

This role will be responsible for conducting in-depth health assessments and evaluating patient needs. Providing pharmacological and non-pharmacological management for patients experiencing behavioral and psychological symptoms of dementia. As well as making appropriate cost-efficient referral decisions. In addition to the outpatient work, this role will perform inpatient geriatric consultations which include diagnosis, treatment, and ongoing follow-up.

The next member of our team will work in a multi-disciplinary inpatient team, to help oversee the implementation of the Age Friendly Health System and ongoing operations as well as measuring process & outcome measures.
