Teaching opportunity

Join a private physician owned group of 19 Physicians including Internal Medicine, Family Practice and Psychiatry plus 13 APP’s in Private Practice providing SNF services to patients on the north side and near west suburban areas of Chicago.  Recruiting for full time SNFists to provide care to patients being admitted to skilled nursing facilities to manage the quality and efficiency of care.  

This is 100 % SNFist with option to work with Residents and Fellows.
Option for Medical Directorship and leadership if interested.

Details:
• Oversee the quality of care of a post-acute care network of SNFs affiliated with a major Health Care System
• Monday – Friday full time schedule with on call once / month. 
• Primary responsibility to provide quality care patients to patients admitted and discharged from client hospital
• Potential for teaching med students/residents and RN staff at client hospital(s)
• Report to CEO, who is the Assistant Professor at the UIC School of Medicine

Qualifications:
• Superior communication skills with patients and families and peers
• Commitment to quality standards of care and QAPI activities
• Demonstrated command of WEB based Clinical Integration (EMR) system
• Board eligible/certified and acceptable academic record
• Leadership experience in post acute care setting preferred
• Experience with working with Case Managers, Discharge Planners and Medical Records 
• Commitment to Geriatric medicine
